Why We Require a Surgical Consent Form

This form ensures that:

  • You understand the nature of your pet’s surgical procedure
  • We have your permission to move forward with treatment
  • You’re informed about potential risks, benefits, and aftercare instructions

Your pet is scheduled for anesthesia and/or surgery today. To ensure that your pet has the safest and most comfortable anesthetic experience possible, we recommend the following:



We additionally recommend placement of an IV catheter with fluids to maintain your pet's blood pressure during surgery and to ensure rapid administration of IV medications in the unlikely event of a complication.


Patients recover more rapidly and comfortable from surgery if they are placed on pain relief medication early in the post-operative period. Your pet may or may not require this medication. The fee ranges from $16-$55, depending on the type and amount of medication used.


Pets who are lost are more likely to be found with a microchip.
